Depends on the form.
I put M and Mr. on them all anyway. They wouldn't let me have the M on my driver's license or passport, though. Everything else, nobody's said anything, and I am recorded as being Mr. Doc, a male.
In my state one gets the impression that a person needs genital surgery to have gender markers on ID changed, but when it comes down to it you just need a note from a doctor that says something along the lines of "I, Doctor So-and-So, have examined Werewolf and say he's male." There's a form for it at the DMV. It does not require the doctor to use any set of criteria to determine your gender. I will get that soon, when I can afford to get yet another new driver's license and passport. Lots, probably many years, before I get genital surgery. If I ever get genital surgery.
Don't worry about it. I've been running around with ID that shows me with a beard, but says I am female, for a little over a year now. Nobody notices. I keep expecting somebody to comment about it, and have my planned response, (something like "Yeah, it's a mistake, and I need a doctor's note for them to fix it, crazy, huh?") but nobody's said a word. Not even when they made copies of my ID for a job.
